After talking on the phone, you will receive an email that links to our patient forms. We ask that you fill out this information completely to the best of your knowledge. This allows your first visit to go smoothly without any delays. Within these forms we will ask for your insurance information so that all of our information is current and up-to-date so there are no billing surprises.

First, we perform a comprehensive exam of your mouth that includes full mouth x-rays, measurement of your gum tissue depths, intra oral teeth photos, oral cancer screening, and saliva testing. We believe this comprehensive exam provides the most complete information we need to offer you a personalized treatment plan moving forward. Second, we clean your teeth by removing plaque and tartar along with flossing and polishing your teeth for a healthy and bright smile. **If periodontal disease is discovered in our comprehensive exam, we will not be able to clean your teeth on your first visit.

We offer the best quality personalized care in the industry, and we want to clearly communicate your personalized treatment plan moving forward. Dr. Maze will discuss with you, in detail, the findings of your exam, and we will agree on the next steps forward.

There are three main treatment programs. Our preventative program, our restorative program, and our periodontal program. Our goal is to get you on the right treatment plan to help you restore and maintain your beautiful smile.

Our preventative program includes (2) routine visits within a calendar year to ensure good overall health. We will clean and and polish your teeth twice a year, and perform a comprehensive exam one time a year to ensure your mouth stays healthy.

Our restorative program is patient dependent based on our comprehensive exam, our goal is to restore your teeth and eliminate pain as quickly as possible. We want you to experience a healthy, pain-free, confident smile as quickly as possible.

Our periodontal therapy program is designed to restore your gums back to health. We must address this problem before we are able to provide further treatment. Our goal is to eliminate gum disease and restore your gums back to good health. Most patients start our restorative plan after this treatment is completed.
